  • This form is for students who have been chosen for verification based on their FAFSA results.
• Special Circumstances Form
  • This form is for students who have special financial circumstances to be considered.
• Summer Financial Aid Form
  • This form is for students who are interested in receiving financial aid for summer classes.
• IRS: Request a Free Tax Transcript
  • Use this form to request a free tax transcript if you have misplaced your tax returns.
• Endowed Scholarship Application
  • This form is for full-time, undergraduate students who wish to apply for additional scholarships.

Work Study Forms

• Transfer Request Form
  • This form is for students currently participating in the Work Study Program that are interested in transferring to a different department. Before completing the form, a student must have approval from the Work Study Coordinator.
• Work Study Handbook
  • The Handbook provides students with information about policies and procedures related to the Work Study Program.
• Work Study Application/Handbook Certification  - Submit Online or Print and Mail
•  Work Study Check Disbursement Form - Submit Online or Print and Mail 

Please print and return the following forms to Donna Price in Human Resources.  A copy of your driver's license and your social security card or birth certificate is required.  These items can be mailed along with the I-9, K-4 and W-4 forms to 210 Lindsey Wilson Street, Columbia, KY 42728.
